For your convenience, Dr. Neel has provided online access to some of the patient information forms you will need to complete in order to begin psychotherapy with her. It is provided in PDF format for printing so that you may complete it ahead of time and bring it with you on your first visit. If you prefer, you may complete the forms prior to your initial visit and send it to Dr. Neel via U.S. mail, fax or e-mail.
